2020-2021 Boys Soccer Return to Play

Below you will find a detailed description of how to fill out the necessary forms in order to participate in athletics at Central High School. Video link: here.

Step 1: Get to the Central High School webpage and drop down the “Athletics” tab.

Step 2: Once the tab has dropped down, click on the “Athletes Sports Clearance” tab.

Step 3: This will take you to the “Sports Clearance Process Page,” which shows the sports offered, and a brief “how to” near the bottom of the page.  On the right hand side of the page, you will see the necessary hyperlinks of the forms needed to return to play. In order of completion, you will need to 

1. Fill out the “Central Unified Return to Activity 2020-21-Central High School” form

2. Fill out the ‘Family ID” form

3. Fill out the “Sports Clearance Packet” AND RECEIVE A SPORTS PHYSICAL

Step 4: Drop off completed “Sports Clearance Packet” at Central East High School’s main office drop box located at 3535 N Cornelia Ave, Fresno, CA 93722.
